Job description:
A Clinical Pharmacist in the Center for Medication Policy is responsible for following the principles of evidenced-based medicine and patient focused pharmaceutical care in daily work activities.

Duties and Responsibilities
Pharmacists in the Center for Medication Policy provide services to improve drug usage and therapeutic outcomes. This is achieved by collaborating with peers across the hospital to develop policies and procedures that impact the medication use process. This new position will support activities that impact the formulary process by working with our formulary evaluation teams, track and develop initiatives to ensure REMS processes are being implemented and followed, collaborate with other teams to manage drug shortages, promote safe use of medications by collaborating with pharmacy informatics, maintain the Center for Medication Policy webpage, participate in the development of medication use evaluations, and provide accurate and timely responses to drug information requests. Additionally, this pharmacist may participate in research, as it relates to medication policy that impacts the delivery of quality care and contribute to student and resident education. Opportunities exist to practice in an adult outpatient clinic on a select schedule. Lastly, we work closely with peers to achieve departmental and divisional goals.

Job Requirements – Education, Training, Experience:
Doctor of Pharmacy Degree. Completion of an accredited clinical residency program preferred or two years of clinical experience or acceptable board certification, and a demonstrated ability to achieve positive patient outcomes in a similar environment.
